update: after further testing, I've learned that:

1) it is NOT necessary to make the collision object children of the main model.
2) I got best results when the driving surface was one convex mesh. For some reason, trying to drive across the tops of multiple collision meshes caused problems.
3) although the document above seems to say that the collision objects can all share the same name, the only time I could get anything to work was when each of the collision meshes had a unique name. COL_BOX_BRIDGE_01 (02, 03 etc)

This is because the center point of the mesh is critical, and when many meshes share the same name, the center point gets placed at the center of the collective, not at the center of one mesh.
